If you are a new patient to our office, you will need to complete new patient paperwork and medical questionnaire at your first appointment.

Payment Options

For your convenience, we accept cash, personal checks, money orders, visa, and mastercard. Payment is expected at the time services are performed. 

Insurance

Our office is a non-assignment office, meaning we do not direct bill your insurance provider. You are responsible for any fees associated with your treatment on the day of treatment. Our administrative staff will gladly aid you with the completion of any required insurance forms, and we can often submit the insurance claim forms on your behalf electronically. Your insurance provider will then re-imburse you directly for the amount agreed upon by your insurance benefits. You should be aware that different insurance companies vary greatly in the types of coverage available. Our staff will gladly submit a pre-treatment estimate to your insurance company so that you will know what your benefits will be prior to initiating treatment.

Patient Referrals

If you are a dentist or dental specialist and have a patient you would like to refer to our office, please contact us at 505-848-5131. If you are a patient and have a family member or friend you would like to refer to us please submit an online appointment request and our scheduling coordinator will contact them to setup an appointment
